How to Manage Swelling After ACL Surgery

If you are a few days out from ACL reconstruction, the swelling in your knee is probably the thing you notice most, and it is also the thing you have the most influence over in these first weeks.

Why Swelling Is the Problem to Solve First

Almost everything in the early weeks after ACL reconstruction traces back to how much fluid sits inside the knee. The surgery itself is reliable and the graft is in place, but the joint responds to the procedure by filling with fluid. That fluid is what you are working against every time you elevate, ice, and compress, and bringing it down is the single most useful thing you can do at home.

Swelling does not only make the knee feel tight and heavy. It suppresses the muscles around the joint, and the quadriceps is the one that matters most. When fluid builds up inside the knee, the joint sends signals that switch the quad off, a response clinicians call arthrogenic muscle inhibition, or AMI. A quad that will not fire cannot do the quad sets and straight-leg raises your physical therapist assigns in the first week, and those early exercises are the foundation everything later is built on.

Swelling also keeps the knee from straightening. Getting the knee to lie flat is one of the first goals your care team sets, and a joint full of fluid resists it. How to Reduce Swelling After ACL Surgery

Elevating Your Knee After ACL Surgery

Elevation uses gravity to drain fluid out of the knee, and it works only when the leg is above the level of your heart. Propping your foot on a coffee table while you sit upright is not high enough. Lie back and raise the whole leg so the knee sits above your chest, with the ankle higher than the knee.

The detail that trips patients up is where the support goes. Stack pillows under the calf and heel so the entire leg is supported and the knee stays straight. Do not place a pillow directly under the knee. A pillow under the knee bends the joint, and holding it in that bent position for hours works directly against the full extension you are trying to win back. Support the leg from underneath, keep it straight, and let the heel be the highest point.

Elevate whenever you are resting, and especially after physical therapy or any stretch of time on your feet, since activity pushes fluid back into the joint. While the leg is up, add ankle pumps by flexing your foot up and down. The calf muscles act as a pump that moves fluid back toward the heart, and pairing that pump with elevation clears the knee faster than elevation on its own.

Cold lowers the temperature in the tissue, slows the inflammatory response, and quiets the nerves that carry pain, so the knee both swells less and hurts less. Gentle movement is part of swelling control, not something separate from it. Ankle pumps, quad sets, and the range-of-motion work your therapist assigns all drive the muscle pump that moves fluid out of the leg. A leg that stays still all day lets fluid pool. Whatever rule your care team sets on weight-bearing and activity comes first, and within those limits, the small movements you can do at home keep the pump running between physical therapy sessions.
